Instructions

  • Use the following form to recommend the purchase of any networked resource that is to be paid
    with central funds.
  • This form cannot be submitted if the required (*) fields are not filled.
  • Do not use this form to request the purchase of stand-alone or locally networked CD-ROM products. (To order stand-alone or locally networked CD-ROM's use the Monograph/Firm Order Request Form or send request directly to the Acquisitions Department by campus mail.)
