Media Center Reserves Form

Use this form to request up to 20 items to be placed on reserve at the Media Center on Douglass Campus. Also use this form to place videos on reserve at the smaller viewing facilities (20 students maximum) in Alexander Library and Kilmer Library. If you're submitting more than 20 items for the same course, please fill out an additional form. To place materials on reserve for Music Department classes, please use the
Performing Arts Library Reserves form.

Bring personal copies of DVDs or videotapes to the Media Center Desk; identify them by instructor's name, course name, and course number. Include the following information: (1) the viewing location (Douglass Media Center, Alexander, Kilmer) you prefer [Douglass has the largest and most up to date facility], (2) course name and number, and (3) the semester your course will be taught.

Please prioritize your requests by week of the semester, it will help us manage workflow and ensure materials are available when students need them. You can note the dates desired in the text box for the reserve items.
