If you are submitting more than 20 items for the same course, please fill out an additional form. This form allows you to place materials on reserve at one library. If you wish to place reserves at more than one library location, you will need to fill out one additional form per library. Bring personal copies of books and photocopies of articles to the Reserve Desk; identify them by instructor's name, course name, and course number. Send electronic files as email attachments (one file per email message) to the appropriate contact (as listed in the contact section below), or drop off a CD or USB/flashdrive at the Reserve Desk.
